Chord Symbols with "extended notes" Stacked in Prentheses

Is there a way to create chord symbols with "extensions" shown after the chord, between parentheses, and on two separate lines, as shown below? I have the jazz fonts installed, and they do a good job of showing jazz chords such as "A7 (flat 9 sharp 11)." However the "extended notes" are on one line, and the result is a chord that takes up a lot of horizontal space in the chart).
