Chords and ties/slurs

I'm having issues with writing a score. Time sig is 4/4 Instrument is a piano. I can't chord a half note and a quarter note in the same chord when holding the one note for 2 counts while the other notes change in the same measure that ties to another quarter note in the following measure. I will leave an example of what I'm trying to do below any assistance will be appreciated.
