There's problems in 'Fractional code entry' and 'Lyrics entry'

First one, in the 'Fractional code entry'
I entered ' B/D# 'code and these letters should be on the same line.
But as you see uploaded image, these're placed sideling.

Last one, in the 'Lyrics entry'
I entered ' - ' on the note which arrow points,
then this didn't placed on the right position.
And that bar wasn't accepted independently, was included in the front letter.
