I have to use 'preview' to see if the file is attached
and [inline...] to insert it
Edit: it seems impossible to delete the attachment
The problem is still there. Can only see the uploaded file in preview, and there is no "Insert" button. Pressing "Remove" causes an error.
@shoichi @geetar when would you say this problem started to occur?

I uploaded an image to the handbook on Weds 23 May and everything worked OK then.
Problem was resolved, thanks for reporting!
