Custom key signatures are currently visual only. They are not understood, therefor don't influence playback, nor are taken into account for courtesy accidentals.

I can see how that would cause the issue I described, but I don't understand what you mean about playback. Custom key signatures are observed when notes are entered, and when you add a custom key signature to an existing measure, notes following it have their accidentals updated appropriately. Since the notes have the appropriate accidentals, they play back as expected.

Either you're using different key signatures and we're not talking about the same thing (or not an official release) or the handbook needs updating. Straight from the handbook page at https://musescore.org/en/handbook/3/key-signatures#custom-key-signatures :
Note: Unlike standard key signatures, custom ones do not affect playback and cannot be transposed in the usual way.

Transposition does not work, but in fact playback does, at least they did the last time I checked. Seems someone implemented this "under the radar", might have been https://github.com/musescore/MuseScore/commit/88703a32595ea41cf5e5cfbcc…?

Happened during initial groundwork for 3.x by werner, which explains why it was missed in release notes. Updated the handbook statement to reflect this.

So what’s the recommended action here? Should I file a feature request, or is there an existing one I can upvote or something?

There's #7440: Displaying custom key signatures in multiple clefs
But feel free to amend it or open another one for the courtesy bug

that thread doesn't seem to have a solution either and just links to this one.

Jeetee said the linked issue did not mention courtesy accidentals and an amendment or new issue would be needed, preferably the new Issue.
Your comments are unrelated to the issue Jeetee linked to. To add a key signature of any kind to a singe staff, press ctrl (Mac cmd) while applying the key signature. This works no matter which method you are using to apply the key signature.
