Note Skipped in Triplet

I have a bar that has 2 consecutive quarter note triplets at the beginning. The 3rd note (beginning of 2nd triplet) is skipped on playback. I've tried converting the skipped note from a dotted quarter to an undotted eigth but it still happens. The attachment shows this problem.

I tried to scan for a similar issue but I couldn't find any so - my apologies if I'm being redundant.

You have set the note velocity for that note to 2 which is barely audible indeed as the possible range for that is [0-127]
