how do I do this "parallel line" thing?

See attachment. I'm trying to do something like that and I can't figure out how to get the parallel pickup notes in there. It seems to want to replace the half note (diamond head). I don't need the parentheses and the "2nd time" necessarily, I can create a second ending for that if I need to.

While I'm at it, which note head would be closest to that traditional jazz one for indicating chord/comp timing?
