The group of notes is a triplet. The square liney things are a bracket. Triplets sometimes just have a 3 above them, sometimes have a bracket or (in older music) you might see a 3 and a curved line that looks like a slur.

You're not following the manual's instructions correctly. To fix, click on the Ab on beat three. Click on the quarter note in the toolbar. Choose Add Tuplet->Triplet from the menu. Now you have a triplet with one eighth note and two eighth rests. Add in your two sixteenth notes to the tuplet.
Or erase and do it from scratch per the manual's instructions (i.e. add a quarter note, turn it into triplets, add following notes as desired.)
