You mean a font for the notes etc... not a soundfont right?
You will need a font editor https://en.wikipedia.org/wiki/Font_editor and you will need to understand the SMuFL standard http://www.smufl.org/. You'll also need patience and persistence because font crafting is an art, and you need time and practice to master it.

It is extremely difficult to do, but possible if you have persistence, as lasconic said.
If you want to use with MuseScore it is difficult as well, but seems to be possible.
