I don't know what's going on...
But every time I insert measures before either of the last two measures of the sample score the software crashes.

Steps from scratch:
1) Grand staff template
2) Clef change let's say measure 4 top staff
3) Insert measure ("Ins" shortcut, or via menu) from this same measure 4

Maybe related to ? #280814: Crash when adding new time signature in the measure of a clef change
What does frequency mean... and what does once mean...

May be confusing, agree, but Frequency is not related to Reproducibility (marked as Always, that is the case here), but to number of reports of the same issue.

Ah I see ;)
Thanks cadiz for linking to that other issue...this is indeed a duplicate.
Fixed by my PR (https://github.com/musescore/MuseScore/pull/4517) for issue #280814: Crash when adding new time signature in the measure of a clef change . I've just tested cadiz's reproduction steps to confirm.
Thanks for checking. Kill two birds with one stone. Cool :)
But for my score it still exists with the newest nightly cf1f5ce and I've tried whole lots of scores.
As said, it's a duplicate. So please, we have to wait the merge of the created PR for the next update.

Hmm... Fine.

And meanwhile, as workaround to avoid the issue: remove the two clef changes -> insert measure(s) -> and re-insert clef changes then.
