The playback speed was working just fine, then without making any edits to the score whatsoever the playback started running at double the marked tempo.
You have two tempo marks, one is pesante with a tempo of 120, the other is the quarter note = 75. MuseScore is quite confused about what you expect. Delete one of the tempo marks and edit the other so it says
Pesante
Q = 75
(Q is the quarter note symbol) this will tell MuseScore exactly what you want.

You need to attach the score, I've never heard of this.
It's marked at 75 bpm but the playback tempo suddenly jumped to about twice the speed.

I'm not seeing the 120 tempo marking when I open it. Where is this showing for you?
That "Pesante" is a tempo text too.
In the attached I combined those 2 into just one
If you select the Pesante and look in the inspector (press F8) you will see the tempo is set to 120, which is the default.
