4.x-dev: numerous issues

See the attached file, "test_piece", for confirmation of the following issues:

There is no Standard file operations toolbar displayed.

When reopened, the program does not remember the last user resize of the program window; it seems to reset to factory default.

When reopened, the program does not remember the last user choice of tabs; it always resets to the default.

The "Would you like to try our improved score style" window opens even if the score style was already updated in MS3. If you close the window using the X icon (or F4), without pressing any selection buttons, the program reports an error (In MS3, the window closes and the score stays unchanged).
Worse; if you choose not to "update" the already-updated file, the program reverts most of the text styles to "free serif" and resets the font-sizes as well.

The program is resetting previously customized note beams; see measures 4 and 12 in the attached example. Perhaps it is doing the same for other customized properties? Expected behaviour is for all customizations to be retained.

The "Open recent" list is not displaying the correct file name but instead shows the "Title" of the piece (the one displayed in the opening vertical frame). Even if you change the file name the "Open recent" list will still show the "Title".

The program does not seem to remember the last opened location; it always defaults to the "Scores" folder when you try to open or save files. Expected behaviour (as MS3) is that it should remember the last opened/saved location.

The sidebar width cannot be changed although it displays an "adjust" icon when you hover over the edge.

The overwhelming majority of palette icons are tiny to the point of invisibility.

The menu options have an ampersand inserted before the name; &File, &Edit etc. Ampersands have also been inserted in many of the menu names and items:

First system Indents are being reapplied even if none was specified in 3.6.2; the style menu, however, continues to show no indentation and there seems to be no way to remove it.

It looks like preferences are not being saved. When I change to "UK Qwerty", layout, it resets to "US Querty" on reopening.

Playback can hardly be heard even with PC volume turned up to maximum.

Playback has been transposed a whole tone higher than concert pitch.

Neither Piano nor Synthesizer can be opened.
