In the piano part, quaver beats 5 and 6 of 12 in bar 48.
Demisemiquavers across bass and treble staff (technically part of the bass staff) appear as semiquavers or quavers.
It seems to be affected by the width of the bar. When the bar is made wider it appears correctly.
It has likely something to do with the stem of the D (beat 6) appearing before the stem of the preceding C (beat 5.75).

Edit: Added score with issue. Note that I tried to recreate the issue with a fresh score, but musescore didn't allow the stems to betray their correct order, which is probably what should happen. Issue should hopefully be at bar 48, on page 24, just before rehearsal mark [K].

Just done that, thanks for the comment. Thanks for fixing the metadata as well, I wasn't entirely sure how everything should be labeled.
Confirmed.
The horizontal spacing problems have already been dealt with (but don't expect them in the patch release coming up, they'll likely go into 4.1). The beam problem has been logged on Github. For the moment, there is a temporary workaround that will solve the issue: place a staff distancer (you'll find it in the layout palette) that matches exactly the current distance of the staves (see screenshot). Or, another more drastic solution: disable vertical justification (Format->Style->Page).
