If you are asking how to create such a score from scratch (i.e. starting with an empty score and entering the notes) then follow the instructions in this handbook article, using the option to choose two treble clef instruments and two bass clef and one percussion. https://musescore.org/en/handbook/4/setting-your-score#overview
If you are asking how to create a condensed score from an exisiting full score - that is an art requiring decisions on what to include and what to leave out.
