Not possible to add a new voice over a voice with tuples
For example, if I create a voice in 4/4 with 4 x 3 ternary 1/8, it's not possible to create a new voice.

Please describe exactly what steps you do and then what goes wrong.

Do you mean something like this?

Make sure that the toolbar note input is in (default) step-time mode.
Second voice: press N to enter note input and Ctrl+Alt+2 to switch to voice 2.
