Probably you accidentally moved the mixer off screen, or changed monitor resolutions since the last time you used it, and the previous position no longer exists. The "Revert to factory settings" will fix this. It's also possible, though,m that you might be able to find some small corner of the window along the edge of the screen and drag it back. Or use a keybaord shortcut to move the window if your OS supports it.
