JACK MIDI Out doesn't send program change message

Unimplemented feature. Commit ba5a316 (merged igevorse/jack-transport branch)

Reproduce:
1. Open your score.
2. Connect MuseScore via JACK to external synthesizer.
2. F10 to open mixer window, change sound (patch/program) of your instrument.

Expected behavior: changing "sound" of instrument in mixer window should update sound in external synthesizer.
Actual behavior: no changes in external synthesizer (set to default "Yamaha Grand Piano" in Qsynth for FluidR3_GM.sf2).
