Append measures?
To be clear: insert means, add before the selected measure. You want "append" (Add / Measures / Append...)

I actually didn't know I could add measures after the last before I saw this post :D
RTFM ;-) see https://musescore.org/en/handbook/3/measure-operations
Insert one measure (Ins) - menu works, but Ins keyboard - doesn't. Same with "Insert measures (Ctrl+Ins)" and "Augmentation dot (.)"

What program and OS settings can affect this?

I have attached the score. What's wrong with it?

Nothing. It just works.
As this is a shortcut action, it could be that your keyboard(driver) isn't sending a normal Insert keycode when attempting this.
Go to Edit → Preferences → Shortcuts and assign an additional shortcut to the command (Ctrl+Shift+K is available by default for example). Does inserting measures work then with this new shortcut?
Also, what shows as keystroke combination in that field when you try to assign the Insert-key to it?

Thank you, I have found the problem.
"Num lock" was on, so "Ins" worked as "0"

Oh, you were talking about NumPad Ins, not the 'normal' Ins key!

It's interesting that no need to set additional "NumPad+Ins" setting - NumPad Ins works withput it.

You must press the correct key combination to get insert from the number pad. If num lock is on, then shift+0 will get you an insert, if num lock is off it will get you a 0. I keep num lock on and have certain items tied to the keypad numbers, but the short cut I have defined as alt+shift+3, MuseScore calls it NumPad+Alt+PgDown or something like that. Needless to say, accidentally hitting the Num Lock key has caused me problems in the past, but my current keyboard puts a giant alert on my screen I can't miss :)
