Problem with Da Capo al Coda structure

Once again, I apologize if I should have figured this out on my own, but I did go through all the resources and still could not find anything really applicable.

My attached song uses the Da capo al Coda structure, which makes it necessary for the G note at the end of measure 12 to be tied with the G at the beginning of measure 16 when the song is repeated, but I could not find anyway to make that happen. Advises would be greatly appreciated.
