Two Notes Playing As Interval (simultaneous) Despite Written As Individual Notes
This is driving me mad as I have no idea why, but the first two notes of the first and last bars are playing as two note intervals instead of consecutive notes.

It's sounding simultaneous notes in the indicated bars on my computer.

Not for me, either.
Try listening with the metronome turned on during playback.
