Location of notes is not correct
Hi, I'm using MuseScore version 3.6.2.548021803, revision: 3224f34
See file: The location of the upper notes is at least confusing. It is not clear whether they are on the lines or between them.

I'd bet they have been manually off-set. Select, press Ctrl+R

I didn't even know that manual off-set was possible...
I use mouse input. I've had this exact thing happen. It can be easy to accidently double click a note, which puts it into edit mode. If you happen to move the mouse after that, the note will become offset. I didn't realize I was in edit mode and dragged the note to a different place. But to MuseScore the pitch of the note was still the original location.
We just have to be careful.

I use mouse input too, exclusively and since 10 years. Never ever had this (accidentalö double-click) happening to me (never unnoticed at least).

There is a thin piece of steel inside the mouse button (which makes the click) and it breaks down after a while and can double click instead of a single click, especially in those produced with poor quality materials (some branded ones also use poor quality).
After buying a silent mouse, I realized the poor quality of the ones I used before.

I might guess that the double-click speed settings in one's OS could be a significant factor. If it's set to register as a double-click only if you do it quick;y, you're less likely to accidentally invoke this than if it's set to accept any sequence of two clicks within a second or whatever. Because I can totally see someone first selecting the note (not really necessary), and then clicking it a second time to initiate the drag.
